  • From I-405 southbound, take NE 8th exit, eastbound
  • From I-405 northbound, take NE 4th exit, follow signs to Hospital
  • Turn north from NE 8th onto 116th Ave. NE
  • Turn left into Hospital campus at the emergency entrance
  • Parking garage is on your right
  • Patient drop off is at entrance in the circular drive on the left
  • Take elevators to the 3rd floor and follow hall on right to Suite 305

  • From I-405 Southbound: Take NE 124th St. Exit (#20)
  • Right onto NE 124th St.
  • Right at 1st stoplight onto 116th Ave. NE
  • Right on NE 128th St. Go through 3 stoplights.
  • Bear left at the top of the hill
  • From I-405 Northbound: Take NE 124th St. Exit (#20B)
  • Stay right and follow sign toward Totem Lake Blvd.
  • At light, road turns into 120th Ave. NE
  • Stay on 120th Ave. NE up a hill
  • Right at NE 128th St.
  • Bear left at the top of the hill
  • Parking: Left at the stop sign into Evergreen Surgery Center Parking
  • Down one level to Level “D”
  • Park in Patient or Handicapped space
  • Evergreen Surgery and Physicians Center entrance is at the corner
    of the garage (opposite the elevators)
  • You will be on the 4th floor of the building when you enter
